Please fill in this form if you have a death to report for a
member. This form will come to the Department email once you hit
submit. If you don't know the date of the death but do know that is was is 2025, then just let us know that.
If you complete this TAPS form, you do not need to mail a copy to the Department Chaplain and the Department Treasurer.
